For most patients, he performs an endoscopic brow lift — a minimally invasive approach that uses several small incisions hidden within the hairline rather than a single long incision across the scalp. Through these incisions, he uses a thin endoscope to visualize the underlying tissues, then carefully releases and repositions the brow to its natural height and secures it with internal fixation. The result is precise elevation with minimal scarring and a shorter recovery.
For patients whose aging is primarily at the outer brow — a common pattern, especially among those who have maintained the central forehead with Botox — Dr. Nichter may recommend a temple lift. This focused procedure uses a single small incision in the temple hairline to lift the lateral brow, opening the eye and softening the outer brow area without affecting the central forehead. It can be performed under local anesthesia in the office.

Consultation and Planning
Dr. Nichter examines your brow position, forehead musculature, skin quality, and the relationship between the brow and upper eyelid. He determines whether the concern is brow descent, excess eyelid skin, or both — and recommends the appropriate technique. You leave with a clear understanding of the plan.
The Day of Your Procedure
An endoscopic brow lift is typically performed under oral sedation and local anesthesia, though IV sedation and general anesthesia are available. The procedure takes approximately one to two hours.
Dr. Nichter makes several small incisions behind the hairline. Using an endoscope — a thin instrument with a camera — he identifies and releases the tissues that anchor the brow in its descended position, then elevates the brow to its natural height and secures it with internal fixation. No scalp skin is removed. The incisions are closed with small sutures or clips.
A temple lift, when appropriate, is a shorter procedure — typically under an hour — performed through a single incision in the temple hairline under local anesthesia.
If the brow lift is combined with an eyelid lift or other facial procedure, the total operative time extends accordingly, with one recovery period rather than multiple.
Immediately After
You are monitored as the sedation clears. Most patients go home the same day. A friend or family member will need to drive you. You leave with detailed post-operative instructions and direct access to the office for any questions.

Recovery and Downtime
Brow lift recovery is generally quicker than most patients expect, particularly with the endoscopic technique.
The First Few Days
Expect swelling and bruising, primarily around the forehead and eyes. Some tightness across the forehead is normal. Discomfort is typically mild to moderate — manageable with prescribed medication. Keep your head elevated as much as possible, including during sleep. Avoid bending over or lifting anything heavy.
Week One
Swelling and bruising begin to fade. Sutures or clips are usually removed around day seven to ten. Most patients feel noticeably better by the end of the first week, though the forehead may still feel tight. Avoid direct sun exposure, strenuous activity, and vigorous facial movements.
Weeks Two Through Four
Most patients feel comfortable returning to work and social activities within ten to fourteen days. Residual swelling continues to diminish. Light activity — walking, gentle movement — can resume as cleared by Dr. Nichter. Avoid vigorous exercise, heavy lifting, and any activity that significantly raises your heart rate or blood pressure.
One Month and Beyond
By four to six weeks, most activity restrictions are lifted. The incisions, hidden within the hairline, continue to fade and become difficult to detect. Numbness or altered sensation near the incision sites is common in the first few weeks and resolves gradually. Final results are typically apparent at three to six months, once all swelling has resolved and tissues have settled into their new position.
