Facial fat transfer — also called facial fat grafting — is a procedure that restores lost volume to the face using the patient's own fat. Fat is harvested from an area where it is unwanted (typically the abdomen, flanks, or thighs), purified, and then precisely injected into areas of the face that have lost fullness due to aging.
Volume loss is one of the earliest and most noticeable signs of facial aging. As the fat pads that give the face its youthful contour shrink and shift downward, the cheeks flatten, the temples hollow, the under-eye area develops shadows, and the face begins to look drawn. These changes often occur before significant skin sagging, and they cannot be corrected by lifting alone.

It is then purified to isolate healthy, intact fat cells from oil and fluid. Your Fat Transfer Experience
Consultation and Planning
Dr. Nichter assesses your facial volume, skin quality, and the specific areas where fullness has been lost. He identifies appropriate donor sites and discusses how much volume each area of the face needs. If a combination with other procedures would produce a better result, he explains the options. You leave with a detailed plan.
The Day of Your Procedure
Facial fat transfer is performed under local anesthesia with sedation, or under general anesthesia. The procedure takes approximately one to two hours, depending on the number of areas being treated.
Dr. Nichter first harvests fat through small-cannula liposuction from the designated donor site — most commonly the abdomen, flanks, or inner thighs. The incisions are tiny and heal with minimal scarring. The harvested fat is gently processed in a centrifuge to separate healthy fat cells from fluid and oil.
Using fine syringes, he then injects the purified fat into the target areas of the face — cheeks, temples, under-eyes, nasolabial folds, lips, or jawline — in small, precise amounts distributed across multiple tissue layers. This layered technique ensures that each deposited fat cell is close to a blood supply, which is critical for long-term survival.
The donor site may be lightly compressed with a garment to minimize swelling.
Immediately After
The sedation clears quickly. Most patients go home within an hour or two. You will have some swelling at both the facial injection sites and the donor area. A friend or family member will need to drive you home.
Recovery
Recovery and Downtime
Fat transfer recovery involves two areas — the face and the donor site — but both heal relatively quickly. Most patients find the recovery more manageable than they anticipated.
The First Few Days
Expect swelling and mild bruising in the face, concentrated around the areas where fat was injected. The cheeks or under-eyes may look overfilled initially — this is partly swelling and partly intentional slight overcorrection to account for normal fat absorption in the weeks ahead. The donor site may feel sore, similar to a deep bruise. Discomfort at both sites is manageable with prescribed or over-the-counter medication. Keep your head elevated and avoid pressing on the treated areas.
Week One
Swelling begins to diminish. Bruising, if present, fades. Most patients feel well enough to return to work within five to seven days, though some facial fullness may still be apparent. Avoid strenuous exercise and heavy lifting. Light walking is encouraged to promote circulation and healing at the donor site.
Weeks Two Through Four
The face begins to look more natural as swelling continues to resolve. Some of the transferred fat is naturally reabsorbed by the body during this period — this is expected and accounted for in the initial placement. The donor site has typically healed and feels comfortable. Light exercise can resume around week two, as cleared by Dr. Nichter.
Three Months and Beyond
By three months, the surviving fat cells have fully integrated into their new location. The face looks naturally full — not overfilled or swollen, but restored. The donor site is smooth and contoured. Because of the complex physiology of fat, roughly 75% of the final result is visible at six months and the full result at one year. From that point, the transferred fat is a permanent part of your facial tissue.
Results
Natural, lasting improvement.
Initial improvement is visible almost immediately, though the face will appear somewhat overfull due to swelling and the slight overcorrection that accounts for fat absorption. Over the following weeks, the result refines — swelling resolves, and the fat settles into a natural appearance. The final result is typically apparent at three to four months.
The fat cells that survive the transfer process — the majority, in most patients — become a permanent part of the facial tissue. They will age with you naturally, responding to weight changes and the passage of time as any facial fat would. The fat cells that survive the initial transfer become a permanent part of your facial tissue. They live and respond to your body's signals, but they behave like the donor-site fat — so the volume improvement remains, even as your native facial fat continues to age naturally.
