California Breast Lift® in Newport Beach

The California Breast Lift® is a breast enhancement technique developed by Dr. Nichter that combines a minimal-scar breast lift with fat transfer. It lifts and reshapes sagging breasts while adding subtle volume using the patient's own fat — harvested through liposuction from another area of the body. The result is a lifted, naturally fuller breast without the use of implants.

This procedure was developed for a specific patient: one with mild to moderate sagging and modest volume loss who wants improvement but does not want the feel, maintenance, or visibility of breast implants. Many women reach this point after pregnancy and breastfeeding, or with natural aging and gravity over time. They want their breasts restored — lifted, reshaped, slightly fuller — but they want the result to feel entirely their own.

The California Breast Lift® addresses this by using only natural tissue. The fat cells, once integrated into the breast, behave like native tissue — they age with the body, respond to weight changes naturally, and have no risk of capsular contracture, rupture, or the other complications associated with implants.

The California Breast Lift® is not a marketing name for a standard procedure. It is a specific technique that Dr. Nichter developed and has refined over years of practice, combining two surgical disciplines — mastopexy and fat grafting — into a single, cohesive procedure.

The technique works in two stages within a single surgery:

Stage one: The lift. Dr. Nichter lifts the breast using short-scar techniques — typically a periareolar (around the areola) or lollipop (areola plus a short vertical incision) pattern. He moved away from the traditional anchor-pattern incision more than fifteen years ago, in favor of techniques that produce equivalent lift with significantly less scarring. The breast tissue is reshaped and the nipple-areola complex is repositioned to a higher, more natural location.

Stage two: The fat transfer. Fat is harvested through liposuction from a donor area — commonly the abdomen, flanks, or thighs. The fat cells are gently processed and purified, then injected into the breast in careful layers to fill out the upper pole, restore volume lost to aging or breastfeeding, and refine the breast contour. Multiple small injections ensure even distribution and smooth integration with the existing tissue.

A lift alone addresses position but not volume loss. Fat transfer alone adds volume but cannot correct sagging. Together, they produce a breast that is lifted, subtly fuller, and entirely natural in look and feel — with the added benefit of contouring the donor site.

As a Board Examiner for the American Board of Plastic Surgery, Dr. Nichter holds his own technique to the same standard he applies when evaluating the work of other surgeons. Is it right for you

Am I a Good Candidate for the California Breast Lift®?

The California Breast Lift® is designed for a specific patient profile. It is an excellent option for the right candidate — but it is not a replacement for all breast lifts or all augmentations.

You may be a good candidate if:

  • You have mild to moderate breast sagging and want a lifted, more youthful position
  • You have lost breast volume after pregnancy, breastfeeding, or aging and want subtle restoration
  • You have adequate excess fat in another area of the body (abdomen, flanks, thighs) for harvesting
  • You want enhancement that looks and feels entirely natural — no implants
  • You prefer minimal scarring
  • You have a normal mammogram and no recent history of breast cancer
  • You are in good general health and at a stable weight
  • You are a non-smoker and do not use other forms of nicotine, or are willing to stop well in advance of surgery

The California Breast Lift® is not appropriate for patients who want a significant increase in breast size (more than about one cup size), who have severe sagging requiring a full anchor-pattern lift, or who have insufficient body fat for harvesting. In those cases, Dr. Nichter will recommend a standard breast lift, breast augmentation with implants, or a combination — and will explain why.

What to expect

Your California Breast Lift® Experience

Consultation and Planning

Dr. Nichter evaluates your breast shape, degree of sagging, nipple position, skin quality, and the amount of volume loss. He also assesses your donor areas — where fat can be harvested — and discusses how much volume enhancement is realistic given your available tissue. If the California Breast Lift® is appropriate for your anatomy and goals, he explains the procedure in detail. If another approach would serve you better, he'll recommend it.

The Day of Surgery

The California Breast Lift® is performed under general anesthesia and typically takes two to three hours.

Dr. Nichter begins with liposuction of the donor area. Small incisions are made at the donor site, and a thin cannula is used to gently break up and remove fat cells. The harvested fat is processed and purified to isolate viable fat cells.

Next, he performs the breast lift using a short-scar technique — reshaping the breast tissue, removing excess skin, and repositioning the nipple-areola complex.

Finally, the purified fat is injected into the breasts in small, carefully placed deposits. Multiple injection points ensure even distribution, smooth contour, and optimal fat cell survival. Dr. Nichter sculpts the breast to restore upper-pole fullness and achieve the shape and volume the patient desires.

Immediately After

You are monitored as anesthesia clears and typically go home the same day. You will wear a surgical bra to support the breasts. Compression garments are worn over the donor site. You leave with detailed care instructions for both the breasts and the liposuction area.

Recovery

Recovery and Downtime

Recovery from the California Breast Lift® involves two healing areas: the breasts and the liposuction donor site. The overall recovery is less demanding than a traditional breast lift with implants.

The First Few Days

Expect soreness and swelling in both the breasts and the donor area. The donor site (abdomen, flanks, or thighs) may feel more sore than the breasts initially. Prescription medication manages discomfort. Rest with your upper body slightly elevated. Do not lie on your stomach or put pressure on your breasts — the transferred fat cells need time to establish blood supply.

Week One

Bruising begins to fade. Swelling diminishes. Most patients take about one week off work. Light daily activities are manageable. Continue wearing your surgical bra and compression garments as directed. Avoid nicotine — it impairs blood flow to the transferred fat cells and can significantly affect fat survival.

Weeks Two Through Four

Most patients return to full daily activity by the second week. Swelling in both areas continues to improve. The breast shape refines as swelling resolves and fat cells integrate. The donor site contour becomes more visible. Avoid vigorous exercise and heavy lifting.

Six Weeks and Beyond

By six weeks, most restrictions are lifted. Exercise can resume gradually. The transferred fat cells are largely integrated by this point, though the breasts continue to refine in shape for three to six months. A small percentage of transferred fat is naturally reabsorbed during the first few months — Dr. Nichter accounts for this during the procedure.

Results

Natural, lasting improvement.

The California Breast Lift® produces a breast that is lifted, subtly fuller, and entirely natural in look and feel. The improvement is visible within the first few weeks as swelling subsides, with the final shape and volume emerging at three to six months.

The transferred fat integrates permanently into the breast tissue. It behaves like native fat — it feels natural to the touch, ages with the body, and responds to weight changes the way natural breast tissue does. There is no implant to maintain, monitor, or eventually replace.

The liposuction donor site also shows permanent improvement. Fat cells removed through liposuction do not regenerate, so the contouring benefit at the donor area is lasting — provided a stable weight is maintained.

Maintaining a stable weight is important for both areas. Significant weight gain will add fat throughout the body, including the breasts and donor site. Significant weight loss may reduce the volume of the transferred fat.

How is the California Breast Lift® different from a regular breast lift?

A standard breast lift reshapes and elevates the breast by removing excess skin and tightening tissue — it does not add volume. The California Breast Lift® adds a fat transfer component: after lifting the breast, Dr. Nichter fills the breast with your own purified fat to restore lost volume. The result is a breast that is both lifted and subtly fuller, without implants.

How is it different from breast augmentation?

Breast augmentation adds volume, typically with implants, but does not address sagging. The California Breast Lift® addresses both — lifting the sagging breast and restoring volume with fat transfer. It is a more modest volume increase than implants provide (about one cup size or less), but the result looks and feels entirely natural.

How much bigger will my breasts be?

The volume increase from fat transfer is subtle — typically about one cup size or less. The more significant visual change comes from the lift and reshaping, which makes the breasts appear fuller and more youthful even without dramatic volume increase.

Where does the fat come from?

Dr. Nichter harvests fat through liposuction from areas where you have excess — most commonly the abdomen, flanks, or thighs. The donor area is contoured in the process, providing a secondary aesthetic benefit.

Does the transferred fat last?

The fat cells that successfully integrate into the breast tissue are permanent. A portion of the transferred fat (typically 20-30%) is naturally reabsorbed during the first few months. Dr. Nichter accounts for this by transferring a slightly higher volume initially. The remaining fat behaves like native breast tissue.

What if I don't have enough fat for the transfer?

Adequate donor fat is a requirement for the procedure. If you do not have sufficient excess fat, the California Breast Lift® may not be an option. Dr. Nichter will assess this during consultation and recommend an alternative approach — such as a standard breast lift with or without implants — if needed.

Will the fat transfer affect mammograms?

Fat transfer can occasionally produce microcalcifications — small calcium deposits visible on a mammogram. These are generally distinguishable from cancer-related findings on modern imaging. Dr. Nichter recommends a baseline mammogram before the procedure and regular follow-up mammograms afterward.

Is this procedure available elsewhere?

The California Breast Lift® was developed by Dr. Nichter and is his proprietary technique. While other surgeons perform fat transfer breast augmentation, the specific combination of short-scar mastopexy and fat grafting technique that defines the California Breast Lift® was developed and refined in his practice.
