Consultation and Planning

Dr. Nichter evaluates your body proportions, skin quality, and fat distribution. He identifies the best donor sites for liposuction and discusses how much volume and projection your buttocks can realistically achieve based on the fat available and your frame. You leave with a clear surgical plan and a breakdown of associated costs.

You will also have the opportunity to meet with the anesthesiologist before your procedure. We use advanced perioperative medications and a non-narcotic general anesthetic where possible to minimize nausea and speed your recovery. Most patients do not require narcotic pain relievers afterward.

The Day of Surgery

A BBL is typically performed under general anesthesia. The procedure takes two to four hours, depending on the extent of liposuction and the volume being transferred.

Dr. Nichter begins with liposuction of the donor areas — most commonly the abdomen, flanks, lower back, and thighs. This step is itself a contouring procedure: the areas from which fat is removed are sculpted to enhance the overall silhouette. The harvested fat is then purified in a centrifuge to separate healthy fat cells from fluid and oil.

Using fine cannulas, he injects the purified fat into the buttocks through small incisions, distributing it in precise amounts across multiple fat-depth layers. The injection is performed above and within the gluteal fat — never into the gluteal muscle or deeper vessels — for safety and to prevent complications. Fat is placed strategically to enhance shape, projection, and symmetry while maintaining proportion with the surrounding frame. You will be placed in elastic compression garments before leaving the facility.

Immediately After

You will be monitored as anesthesia clears. Most patients go home the same day. You will wear a compression garment over the donor areas. A friend or family member must drive you, and someone should stay with you for the first twenty-four hours.

Recovery

Recovery and Downtime

BBL recovery requires specific precautions that differ from most cosmetic procedures. The most important: you must avoid lying directly on your back for the first several weeks. This protects the transferred fat cells during the critical period when they are establishing a new blood supply.

The First Few Days

Expect soreness at both the donor sites and the buttocks. The donor areas feel bruised; the buttocks feel tight and swollen. Discomfort is manageable with prescribed medication. Sleep on your side or stomach. When resting, use pillows to keep weight off the buttocks. A compression garment is worn over the liposuction areas to reduce swelling. You may shower on the third post-operative day.

Week One

Bruising at the donor sites begins to fade. Swelling in the buttocks is at its peak — the buttocks will appear larger than the final result. You may use a specially designed BBL pillow or cushion when sitting is unavoidable, but minimize sitting time. No driving during week one. Light walking is encouraged.

Weeks Two Through Four

Most patients return to work by week two or three, depending on the nature of their work and the ability to avoid prolonged sitting. Swelling continues to diminish. Some of the transferred fat is naturally reabsorbed during this period — this is expected and accounted for in the initial volume placement. Light daily activity is encouraged, but avoid exercise, heavy lifting, and impact.

Six Weeks and Beyond

By six weeks, most activity restrictions are lifted. You can gradually resume exercise, starting with low-impact activity. Sitting is no longer restricted, though a cushion may still be more comfortable. The buttocks continue to settle and refine over three to six months as swelling resolves and the surviving fat integrates fully. Final results are typically visible at three to six months.

Results

Natural, lasting improvement.

Initial results are visible once bruising and swelling subside — typically by the second or third week. The buttocks will appear somewhat larger than the final result initially due to swelling and the expected reabsorption of a portion of transferred fat. Over the following weeks and months, the result refines as the surviving fat cells establish themselves.

The fat cells that survive the transfer become a permanent part of the buttock tissue. They will respond to weight changes as any fat cells do — growing with weight gain, shrinking with weight loss. Maintaining a stable weight produces the most consistent long-term result.
